As of August 1, 2021 all grantees will use the form below for reporting on all SPN grants. Please fill out the identifying information about your report submission and select the appropriate grant program from the drop down list.
If you are unsure of which grant program to select or if the grant program you are submitting for does not appear on the list, simply select ‘Other’ and our team will make sure your report is assigned correctly.
- To ensure your report is attributed correctly please ensure that your organization’s name and the name of the program you are reporting on are both in file name of your attachment.
As this change temporarily removes your ability to view grant reporting dates and templates, please reach out directly to David Snyder (dsnyder@spn.org) with any questions or requests for information.
As a reminder, grant reporting dates and appropriate template are in the original grant agreement signed by your organization.
